A FEW THINGS TO KEEP IN MIND

On your first appointment

Please bring your driver’s license or government photo identification and insurance card (if applicable).

For your convenience, new patient forms are easily available by text or email. We ask that you arrive 15 minutes prior to your appointment. If you are unable to access the forms electronically, we ask that you arrive a minimum of 30 minutes prior to your appointment.

We require a credit/debit card on file at the time of booking an appointment.

Your card will not be charged to be placed on file.

Cancellation/Rescheduling/No show Policy

At Footsteps to Wellness, we do not double book our patients. We promote value over volume. Each appointment is scheduled just for you. We ask that you call us with 24 hours’ notice of a cancellation or the need to reschedule to avoid a $75.00 cancellation fee.

We continue to observe and follow guidelines set forth from the CDC. For the safety our of patients, visitors and staff, please note that face coverings (covering both the nose and mouth) is required while in the office.
